Participants requested for hospice survey

One of our goals at Hospice of the Rock River Valley is to promote the integration of comprehensive, high-quality, seamless delivery of end-of-life care for individuals, families and caregivers in our community.

We invite community members to participate in a community awareness survey and would appreciate your help in identifying concerns about end-of-life planning and care. Your responses are confidential and will make a real difference.

The survey can be found on the homepage of our website, www.hospicerockriver.org, and will take about 5 minutes to complete. If you would prefer a survey mailed to you, please call the office at 815-288-3673.
