BELVIDERE (AP) — Federal officials say a northern Illinois manufacturing plant ignored safety auditor warnings before a 2009 fatal explosion.
WNIJ radio reports (http://bit.ly/1hJgt6W) the U.S. Chemical Safety Board issued its final report Thursday on the explosion at NDK America Inc. in Belvidere. Debris from the explosion killed a man at a nearby rest area on Interstate 90. The truck driver was walking to his vehicle at the time.
The federal investigation also found a vessel used to make synthetic quartz didn't meet state and local standards.
NDK attorney Gary Vist says the company will study the board's recommendations. The board says the company should update its inspection schedule.
