CHICAGO – Andrew J. Ptak, 37, of Chicago, formerly of Amboy and Rockford, died Tuesday, Oct. 30, 2012, in Chicago. He had worked as a finance manager for Anderson Hyundai in Rockford.

He was born Oct. 15, 1975, in Dixon, the son of James and Isabelle (Howell) Ptak. He married Ardell Cadacio on Feb. 13, 1996, in Glendale, Ariz.

Survivors include his wife and four children, Felicya, Tsjana, Christopher and Nicholas, all of Surprise, Ariz.; his parents of Amboy; three sisters, Paula Richardson of Amboy, Brandi (Joe) Dunphy of Lacon and Tracy Nodlan of Ottawa; one brother, Jamie Heiss of Ottawa; three nieces; four nephews; one great-niece; and one great-nephew.
